It emerges from the posterior border of the sternocleidomastoid muscle, loops around it, and can be traced proximally to identify C4 inferiorly to the C5 spinal nerve. Proximal dissection of the C5 nerve requires the resection of the transverse process with a high-speed drill or a No. Venous plexus around the spinal nerve close to the foramen requires meticulous hemostasis with bipolar, Gelfoam with thrombon, or Surgicel. Several subscapular branches arise, usually from the posterior cord or axillary, and run inferiorly and obliquely, innervating the subscapularis and teres major muscles. The thoracodorsal nerve arises from the posterior cord, innervating the latissimus dorsi.
